- Irreversibly damaged pulpitis – the state in which the tooth pulp is infected and inflamed.
- Necrosis of the pulp tissue.
- Injury (trauma), complicated fracture of the tooth crown, in condition that root apex is already developed.
Root canal treatment procedure at Udental Clinic
For the best result and long-time maintenance, two visit treatment with 7-day interval is applied at Udental Clinic to give enough time for the medicine to have effect as well as the tooth and supporting tissue to heal perfectly before being sealed and filled permanently.
In some cases that patient does not have time and the infection is not so severe, our clinic can also do one visit root canal treatment.
In rare cases that infection is too complicated, the treatment may last more than 2 visits to eliminate perfectly all the bacteria inside root canals and surrounding tissues. The length of treatment is expected and informed customers in advance.
THE FIRST VISIT
Step 1: Anesthesia
- Local anesthesia is administered via injections to numb the tooth to be treated and the surrounding tissues. There will be no pain at all during the procedure.
Step 2: Tooth isolation
- Dental dam will be in place, to isolate infected tooth from the rest of mouth to facilitate root canal treatment.
Step 3: Access tooth cavity
- A small access hole is drilled through the biting surface of an affected back tooth or from behind a front tooth, allowing access to the pulp chamber and root canals for treatment.
Step 4: Cleaning, shaping and irrigation
- The diseased and dead pulp tissue is removed from the tooth with specially designed instruments used to clean out the root canals and pulp chamber.
- The canals are then shaped with tiny flexible instruments, then washed and cleaned again to remove root canal debris prior to sealing them.
- Antiseptic and antibacterial solutions are used continuously to disinfect the root canal system inside the treated tooth.
Step 5: Temporary filling
- Specific medicine is place inside root canals for anti-bacterial effect and temporary filling material will then be placed to seal the access hole that was made to treat the canals.
- Dental dam is removed.
Step 6: Prescription
- After the procedure, an antibiotic may be prescribed to treat or prevent infection. Be sure to follow the instructions carefully.
- After-effects of treatment are minimal, generally lasting from a couple of days to about a week. It is normal to have some minor discomfort after treatment including slight soreness that can usually be managed with over-the-counter (aspirin, ibuprofen) medications or prescription (codeine-type) drugs, or a combination of the two.
THE SECOND VISIT
Step 7: Obturation
- Root canal filling material, called gutta percha, will be placed fitly together with adhesive cement called sealer in the prepared root canals.
- Sealing the canals is critically important to prevent them from becoming re-infected with bacteria.
Step 8: Restoration
- In cases that the tooth lacks sufficient structure to hold a restoration (filling) in place, a post (either metal or a very strong plastic) will be placed in one of the canals to help retain it.
- After root canal treatment, a permanent restoration – a filling or a crown – must be done to replace lost tooth structure, and provide a complete seal to the top of the tooth.
- This step is of particular importance since if the filled root canals are re-contaminated with bacteria from the mouth, there could be a recurrence of infection around the tooth.
Step 9: Assessment of treatment outcome
- Normally after finishing root canal treatment and restoration, there is one appointment for re-examination the recovery of the tooth. In fact, most of cases do not really need this visit since they are all free of pain and at good state of eating with the tooth after treatment.
